RESCHEDULING OR CANCELING AN APPOINTMENT | When you schedule an appointment with us, we reserve that time and prepare in anticipation of serving you. If you should need to change a scheduled appointment, we kindly request that you notify us at least two business days prior so that we may have ample time to contact other patients who are awaiting time for treatment. We understand that conflicts can arise unexpectedly; however failing your appointment or canceling without adequate advanced notice more than once may result in a charge. 

For extensive procedures that require an appointment time of three or more hours, we require a non-refundable deposit of $1,000 which will be applied to your treatment fee in whole on the day that services are rendered.

FORMS OF PAYMENT | We welcome a variety of payment options including cash, check, MasterCard, Visa, American Express and Discover. Payment for treatment is due and payable on the day that services are rendered. For honoring that agreement we extend a 5% courtesy savings to patients who have no outstanding balance and make payment in full by cash or check. We extend a 3% courtesy savings when payment is made by credit card. These savings do not apply to products or cosmetic treatment.

For your convenience, we offer extended payment plans for amounts up to $35,000 upon approved credit. This plan offers the following features:

• No down payment.
• Low monthly payments.
• No prepayment penalty.
• No- to low-fixed interest rates.

DENTAL INSURANCE | Dental insurance is a contract between your employer and the insurance company. The benefits you receive depend on the policy your employer has negotiated and are not necessarily based on assuring that you achieve a high quality result. We realize this can be confusing and complex. Our insurance coordinator is available to review your policy and provide a complimentary benefit analysis, as well as assist you in filing claims to help you to maximize the reimbursement to which you are entitled. We will process all insurance company forms.  Boger Dental may or may not be a provider. The amount of compensation you receive will be dependent upon your coverage plan and specifics.

Cosmetic procedures are generally not covered by most dental insurance policies; however, partial benefits may be available if teeth are broken or have previous dental fillings, bonding or crowns.
